Coronavirus/Covid-19 Waiver
I acknowledge the contagious nature of the Coronavirus/Covid-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge the LEGIT BALLAZZ 7v7 has put in place preventative measures to reduce the spread of the Coronavirus/Covid-19. I further acknowledge that LEGIT BALLAZZ 7v7 cannot guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/Covid-19 may result from actions, omissions, or negligence of myself and others, including, but not limited to, staff, and other clients and their families.
I voluntarily seek services provided by LEGIT BALLAZZ 7v7 and acknowledge that I must comply with all set procedures to reduce the spread while attending this event. I attest that:
I am not experiencing any symptoms of illness such as couching, shortness of breath or difficulty breathing, fever, chills, repeated shaking chills, muscle pain, headache, sore throat, or new loss of taste or smell. I have not travel internationally within the last 14 days. I have not traveled to a highly impacted area within the United States of America in the last 14 days. I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/Covid-19. I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non-contagious by state or local public health authorities. I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/Covid-19.
Liability Waiver
On behalf of myself and any minor children entrusted to my care, I hereby agree to release LEGIT BALLAZZ 7V7, its respective parent corporations, subsidiaries and affiliate corporations, and all of their respective shareholders, members, officers, directors, employees, agents, successors and assigns from all liability for injury, death, property loss and damage arising out of or in connection with (1) my or my child’s participation in any activity related to LEGIT BALLAZZ 7V7 and the use of any facility, equipment, programs and services related to LEGIT BALLAZZ 7V7, (2) my violation of the policies of LEGIT BALLAZZ 7V7, or (3) my negligent or intentional tortious acts or omissions. This release and indemnity agreement include claims based upon negligence by LEGIT BALLAZZ 7V7 and any other person or cause. Having read this waiver and release of liability, and aware that LEGIT BALLAZZ 7V7 only carries secondary liability insurance, I understand any and all medical expenses are my responsibility under my primary insurance. I further agree to pay LEGIT BALLAZZ 7V7 all costs and legal fees expended defending against such claims or lawsuits as well as any sum paid as a result of any judgment or settlement.
I GRANT PERMISSION TO LEGIT BALLAZZ 7V7 AND ITS AFFILIATES THE ROYALTY-FREE AND PERPETUAL RIGHT THROUGHOUT THE WORLD TO USE, PUBLISH, BROADCAST, MODIFY, AND COPYRIGHT, ANY AND ALL CONTENT OF ME TAKEN IN CONNECTION WITH LEGIT BALLAZZ 7V7, INCLUDING MY NAME, IMAGE, LIKENESS, VOICE, OR ANY MATERIAL BASED ON OR DERIVED THERE FROM IN CONNECTION WITH ANY MEDIA FOR PURPOSES OF ADVERTISING, PROMOTING, AND PUBLICIZING THE LEGIT BALLAZZ 7V7 BRAND, ITS PRODUCTS OR ITS EVENTS.
If any provision of this agreement is determined to be unenforceable, all other provisions shall be given full force and effect.
